How to obtain Service and Medical Records:
In an effort to streamline the process of acquiring documents to support claims for compensation through the Department of Veterans' Affairs (DVA), you can apply for copies of your Service and Medical Records on the internet through the Department of Defence website.
If you are a former member of the Australian Defence Force (ADF), or if your are an advocate of an Ex-Service Organisation assisting with a claim, you can apply for these records through the Freedom of Information and Information Management Branch of the Department of Defence.
Army medical records for people discharged prior to 1947, and Air Force medical records for people discharged prior to 1952, are held by the Department of Veterans' Affairs (DVA). Therefore, if your request falls within this period, please contact DVA.
If you are requesting service records of an Australian member who served during World War I and World War II it is recommended that you contact the National Archives of Australia (NAA) in the first instance.
In-Home Telemonitoring:
Around 300 veterans will be able to participate in the trial in the National Broadband Network (NBN) connected areas of Toowoomba in Queensland, Coffs Harbour and Armidale in New South Wales, Mandurah and Geraldton in Western Australia and Kingston Beach in Tasmania, commencing from July 2012.
The $8 million 3 year trial which will enable chronically ill veterans to have their health monitored by health professionals without leaving their home.
Veterans that participate in the trial will have an individual health care plan developed for them in consultation with their GP and a nurse coordinator. Telemonitoring equipment will be installed in their homes and training will be provided. The equipment will measure vital signs and securely transmit the data using the NBN’s high speed broadband. The data will be monitored by a practice nurse coordinator and the veteran’s GP.
The potential benefits to those veterans and war widows participating in the trial include better management of their chronic conditions and less time spent travelling to appointments.
The trial will be closely linked with the Coordinated Veterans’ Care Program, which provides ongoing, planned and coordinated care, led by a GP with a nurse coordinator, to eligible veterans who have chronic conditions, complex care needs and are at risk of an unplanned hospitalisation.
All information will be provided via a secure format and the safety and privacy of personal data will be protected.
